Continuing Connections strives to enhance the quality of life of adults who have a mental illness and reside in a continuing care facility. Client referrals are accepted through staff at facilities. The program connects residents with their community and encourages activity to reduce feelings of loneliness caused by isolation.
The Continuing Connections program is an innovative partnership between Alberta Health Services providing clinical mental health treatment, consultation, support, and education and Canadian Mental Health Association – Calgary Region providing:
- Introduction to foundation skill building group: Art of Friendship;
- On-site leisure and recreation activities such as the peer-supported social group Circle of Friends;
- Assistance with discovering community resources;
- On-site transitional support.
Members of the Continuing Connections team are also available for informal consultation on various issues including; mental health concerns affecting continuing care residents, education seminars for staff at continuing care facilities, use of volunteer resources and more.
