Client Experience Survey
Canadian Mental Health Association (CMHA) – Calgary Region is working to continuously improve based on feedback received by individuals who have participated in our programs/services. Collection of information from the Client Experience Surveys will be used for the purpose of program evaluation and planning. All feedback provided through the Client Experience Survey is anonymous – we are not asking for your name. Data will be aggregated, which means, the collective information will not allow for individual identification.

CMHA – Calgary Region will collect, use, and disclose information about you for the following purposes:
- Collect information to best identify how to improve our services/practices;
- Data used to identify formal and informal education preparation.

There is also an option to complete a manual version of the survey, by contacting our Outcomes Measurement and Reporting (OMR) Coordinator at email@example.com or 403-297-1726. All manual surveys will be returned to our OMR Coordinator to be included with other feedback received.
