The Street Outreach and Stabilization Program (SOS) works to link hard-to-reach unhoused individuals experiencing mental health and/or addiction concerns to essential community services.

Referrals can be received from any professional or can be a self-referral. For a self-referral, clients can present themselves at the Crossroads Centre (316 7 Avenue SE) on a walk-in basis. The client must consent to the referral in all cases.

SOS Referral Form

 

The SOS team supports individuals with the resources needed to help turn their lives around. Supports provided enables unhoused individuals to get back on their feet so they can live in safety and comfort. Services include linkage to:

  • Psychiatric and medical treatments
  • Addiction support and/or referrals
  • Housing referrals
  • Daily living skills

A referral can be received from any professional or can be a self-referral by presenting themselves at the Crossroads Centre and SOS support. The client must consent to the referral in all cases. Once a client is referred, the SOS team will meet with the client to discuss services offered and together develop an individual service plan. The service plan identifies areas of need, as well as outlines the services and activities necessary for the client to meet identified goals and improve their journey to recovery. If the client is not ready for direct service, the coordinator will continue to build rapport with the client through the outreach component until a stronger relationship is developed.
